       Volunteer Services Agreement

I hereby affirm that the information provided on this application is true and complete to the best of my knowledge.  I authorize persons, my current employer (if applicable) and organization names in this application to provide any relevant information that may be requested.  I also agree that falsified information or significant omissions may disqualify me from further consideration and may be considered justification for dismissal if discovered at a later date.

This Release and Waiver of Liability ("Release") is executed on the date below by the undersigned individual ("Volunteer"), in favor of Ellicott Wildlife Rehabilitation Center, a Colorado nonprofit corporation, and its trustees, directors. Officers, employees, and agents (collectively, "EWRC.")

In consideration of Volunteer being permitted to serve as a volunteer at EWRC and perform volunteer functions and assist with events, functions, and activities at EWRC on dates as assigned by EWRC, the undersigned Volunteer for himself/herself, his/her spouse, legal representatives, heirs and assigns, hereby forever unconditionally releases, waives, indemnifies, and discharges EWRC, and agrees to hold EWRC and its successors and assigns harmless, from all liability, claims, and demands of whatever kind or nature, whether at law or in equity, whether caused by the negligence of EWRC, by reason of any bodily injury, personal injury, illness, death, and loss or damage to property that arises out of or may hereafter arise out of events, functions or activities at EWRC or in the performance of volunteer functions at EWRC.

Volunteer agrees that this Release is intended to be as broad and inclusive as permitted by the laws of Colorado and that if any portion of this Release is held unenforceable, the balance shall, not withstanding, continue in full force and effect.  Volunteer, or in the event Volunteer is a minor, the undersigned custodial parent or guardian of the Volunteer further understands and agrees that EWRC does not assume any responsibility for or obligation to provide financial assistance of any kind or nature, including but not limited to medical, health, vision or dental expenses incurred by Volunteer or disability insurance in the event of an injury or illness to Volunteer.

Volunteer Ethics
EWRC is a care facility, not a zoo.  Quietness is very important to keep the stress levels of recuperating wildlife at a minimum.  The utmost in respect and consideration of these animals is paramount.  Handling of these animals is not permitted at any time.  Handling of the animals or inappropriate conduct toward the animals or anyone at EWRC will result in Volunteer termination.

I have read this release and waiver of liability and fully understand its terms, understand that I will give up substantial rights by signing it, and have signed it freely and voluntarily without any inducement, assurance of guarantee being to me and intend my signature to be a complete and unconditional release of all liability.
